Publishing your form

Forms are Draft by default. To make a form publicly accessible:

  1. Open the form builder

  2. Click Publish in the top-right corner

  3. The button changes to Unpublish and your form is live

To take a form offline, click Unpublish. Existing responses are preserved.


Your form URL

Every form has a public URL:

https://forms.wtf/f/your-slug

The slug is auto-generated from your form title when you create the form. You can customize it in Form Settings → Custom Slug (Pro+).

Example:

  • Auto-generated: forms.wtf/f/dao-contributor-survey-k3x9

  • Custom: forms.wtf/f/dao-survey


Sharing your form

Copy your form URL and share it anywhere:

  • Post in Discord, Telegram, or Twitter/X

  • Link from your website or Notion page

  • Add to a token-gated Discord channel (members click the link, verify via wallet)

  • Include in an email or newsletter


What respondents see

When someone visits your form URL:

  1. Welcome screen (if configured) — with your custom heading and start button

  2. Wallet connection prompt — they connect their wallet (or use a social login)

  3. Token gate check (if configured) — eligibility is verified before the form loads

  4. Form questions — one at a time, with a progress bar

  5. End screen — success message after submission

Respondents don't need a forms.wtf account — they just need a wallet.


Sharing a draft (preview)

You can preview your form at any time from the builder by clicking Preview. The preview URL works even when the form is unpublished — but only for you while logged in.


Tracking who shared

forms.wtf automatically captures the referrer URL for each form view. This means you can see which platform (Twitter, Discord, etc.) is driving traffic to your form in the analytics dashboard.


Multiple forms

There's no limit on how many forms you can publish simultaneously (Pro+ plans). Free plan supports up to 5 forms.
