Custom End Screen

The end screen is the thank-you page shown to respondents after they submit your form. By default it shows a generic success message — you can customize it on Team plan and above.

All forms show a success state after submission with:

• A checkmark icon

• "Thank you for your response!" heading

• "Your submission has been received." body

Customizing the end screen (Team+)

1. In the form builder, click the + button to add a question

2. Select End Screen from the question type list

3. Edit the heading and body text

4. Save your form

Only one End Screen is allowed per form. It's shown after every successful submission.

Fields you can customize:

• Heading — the main message (e.g. "You're on the list!")

• Body — additional context (e.g. "We'll reach out via Discord when the allowlist closes.")

• Button label — the button that lets respondents close or go back

Ideas for end screen copy

NFT allowlist:

Heading: "You're on the list!" Body: "Your wallet has been registered. We'll announce mint details in Discord."

DAO onboarding:

Heading: "Welcome to the DAO!" Body: "Our team will review your application and reach out via the contact info you provided."

Event RSVP:

Heading: "See you there!" Body: "You're confirmed. Add the event to your calendar and join our Discord for updates."

Feedback form:

Heading: "Thanks for the feedback!" Body: "We read every submission. Your input helps us improve."

Plan requirement

Custom end screens require Team plan or higher. On Free and Pro plans, the default end screen is shown regardless of whether you've configured a custom one.
