Creating Your First Form

This guide walks you through creating and publishing your first form in under 5 minutes.

Step 1 — Create a new form

1. From your dashboard, click New Form

2. Enter a title for your form (e.g. "DAO Contributor Survey")

3. Click Create — you're taken straight to the form builder

Step 2 — Add questions

The form builder opens with one empty question. Click the question to edit it.

To add a question:

• Click the + button below any existing question

• Choose a question type from the panel (Short text, Multiple choice, Wallet address, etc.)

• Type your question label

To reorder questions:

• Drag the handle icon (⣿) on the left side of any question up or down

To delete a question:

• Click the trash icon on the right side of the question

Step 3 — Configure each question

Each question has these settings:

Setting	Description
Required	Respondent must answer before proceeding
Description	Optional helper text shown below the question
Type-specific options	e.g. choice options for Multiple choice, file size limit for File upload

Step 4 — (Optional) Add token gating

Token gating restricts who can fill out your form based on on-chain token ownership.

1. Click Settings in the form builder sidebar

2. Toggle Token Gating on

3. Add a rule: select chain, contract address, token type, and minimum balance

4. Choose gate logic: ALL rules (AND) or ANY rule (OR)

Step 5 — Publish your form

When you're ready:

1. Click Publish in the top-right corner of the builder

2. Your form goes live at https://forms.wtf/f/your-slug

3. Share this link with your community

Note: Forms are private (unpublished) by default. Nobody can access them until you publish.

Step 6 — View responses

As responses come in:

1. Go to your dashboard

2. Click on the form card

3. Select Responses to see all submissions
