Form Builder Overview

The form builder is a drag-and-drop editor for creating multi-step forms. Each question is shown to respondents one at a time — a conversational, focused experience.


Layout

The builder has three main areas:

Question list (left panel)

  • Shows all questions in order

  • Drag the ⣿ handle to reorder

  • Click any question to select and edit it

  • Click + to add a new question

Question editor (right panel)

  • Edit the question label and description

  • Configure type-specific options (choices, file limits, etc.)

  • Toggle required on/off

  • Delete the question


Saving

Changes are saved automatically as you type. You'll see a "Saving…" indicator in the toolbar. You can also press Cmd+S (Mac) or Ctrl+S (Windows) to force save.


Preview

Click Preview to open a live preview of your form in a new tab. The preview shows exactly what respondents will see, including the progress bar, question transitions, and submit screen.

Preview mode always shows the form even if it's unpublished.


Form settings

Click Settings (gear icon) in the toolbar to access:

Question order and flow

Questions are shown in the order they appear in the left panel. Respondents go through them one at a time — there is no skipping or going back (by default).

Some question types auto-advance (no OK button needed):

  • Multiple choice

  • Yes/No

  • Rating (1–5)

  • Opinion Scale (1–10)

  • NPS (0–10)

  • Statement

  • Welcome Screen

All other types require the respondent to click OK or press Enter to proceed.


Welcome screen and end screen

Two special question types define the form's wrapper:

  • Welcome Screen — the first thing respondents see. Set a title, description, and button label.

  • End Screen — the thank-you screen shown after successful submission. Customize the heading and message.

These are not counted as form "questions" in the response data.
