Form Settings

Access form settings by clicking the gear icon in the form builder toolbar.


General

Title and description

  • Title — shown at the top of the form and in browser tab

  • Description — optional subtitle displayed below the title

Custom slug

Change the URL of your form from the auto-generated slug to something memorable.

  • Default: forms.wtf/f/my-form-title-abc123

  • Custom: forms.wtf/f/dao-survey

Requirements:

  • 3–60 characters

  • Letters, numbers, and hyphens only

  • Must be unique across all forms on the platform

  • Requires Pro plan or higher

Changing the slug breaks existing links. Share the new URL after changing.


Access & gating

Token gating

Restrict form access to on-chain token holders. See Token Gating for the full guide.

Published / Draft

Toggle whether the form is publicly accessible.

  • Draft — only you can see it

  • Published — live at the public URL


Notifications

Email notifications

Receive an email each time someone submits your form.

  • Requires a verified email address in your profile

  • Requires Pro plan or higher

  • Configure the recipient email address (defaults to your account email)


Branding

Remove "Powered by forms.wtf"

Hide the powered-by footer from your form.

  • Requires Pro plan or higher

  • Useful for white-label use cases


Integrations

Google Sheets sync

Automatically write each submission as a new row in a Google Spreadsheet.

  • Requires connecting your Google account in Settings → Integrations

  • Choose or create a spreadsheet for this form

  • Requires Pro plan or higher

Send a POST request to a URL on each new submission.

  • Requires Team plan or higher

  • Configure multiple webhooks per form

  • Supports HMAC signature verification

Advanced

Custom end screen

Customize the success screen shown after submission.

  • Default message: "Thank you for your response!"

  • Customize the heading and body text

  • Requires Team plan or higher

Email verification on forms

When an Email question has verification enabled, respondents must verify they own the email address by entering a one-time code.

  • Requires Pro plan or higher

  • Toggle per-question, not per-form
