Title and description
Title — shown at the top of the form and in browser tab
Description — optional subtitle displayed below the title
Change the URL of your form from the auto-generated slug to something memorable.
Default: forms.wtf/f/my-form-title-abc123
Custom: forms.wtf/f/dao-survey
Requirements:
Letters, numbers, and hyphens only
Must be unique across all forms on the platform
Requires Pro plan or higher
Changing the slug breaks existing links. Share the new URL after changing.
Access & gating

Toggle whether the form is publicly accessible.
